James H. Woodworth Memorial Award Form
The Distinguished Student Award is given to a graduating student (associate or baccalaureate) who has demonstrated specific contributions and excellence in each of three areas: scholarship, extracurricular participation and/or leadership, and community service outside of UMA. The Woodworth Memorial Award honors a non-traditional student. The recipient should be someone who is graduating (associate or baccalaureate), and who would not normally be recognized for academic or extracurricular achievement, but who has achieved a great deal within the context of his or her total responsibilities and earned a GPA of 2.5 or above. Faculty, staff, and other students may nominate students, or students may nominate themselves.
An electronic nomination form for each award can be accessed through the links above and submitted online. A print version for both awards is available through the Office of the Dean of Students. Please nominate as many students as you think may be deserving of consideration for these awards.
The nomination deadline is noon, March 22, 2013. Please submit your completed nomination form as soon as possible to the Office of the Dean of Students in Augusta. Incomplete forms may not be processed.
If you have any questions about these awards, please contact Michele Weymouth, Office of the Dean of Students, at 621-3176/1-877-862-1234, ext. 3176.
