The James H. Woodworth Memorial award will be a non-traditional student, who is graduating (associate or baccalaureate), who normally would not be recognized for academic or extracurricular achievement, but who has achieved a great deal within the context of his/her responsibilities, and who has earned a GPA of 2.5 or higher.
This form must be completed and submitted by noon of March 18, 2024. If nominator cannot supply all of the information, please contact the nominee for the information needed to complete this form.
Faculty, staff, and other students may nominate students, or students may nominate themselves. If a student self-nominates (or another student nominates a student), a letter of recommendation from a faculty or staff member must accompany the nomination.
